Which size do I actually need?

Pick by who is under it, not by the bed it sits on. A velvet sherpa blanket is heavier and warmer than a cotton throw, so most people use the smaller sizes as a lap layer on the sofa and the two largest as a top cover on the bed.

Will the sherpa go flat and matted after a few washes?

Not if you keep heat away from it. Sherpa pile is what makes this blanket warm, and heat is the one thing that ruins it. The velvet face is fine; the fleece back is where damage shows.

The velvet face is smooth and dense, which is why it drapes rather than tents over your knees, and why hair and lint brush straight off it instead of clinging. The sherpa reverse is looped and lofted, and that loft is the actual insulation: air trapped in the pile, not sheer thickness. Turn it velvet-side up on a mild evening, sherpa-side down when it's genuinely cold. It also means the blanket looks presentable folded over an arm of the sofa in a way that a plain fleece never quite does.
